Yes you have to refrigerate, I have never canned milk. But maybe if you pressurized it you could keep it on the shelf. We usaully only make a lot close to christmas or in the spring and give a lot away as candy and goodies. But I am guessing at least 2 weeks since it has so much sugar and is cooked for a while. Sorry I can't be more specific, you can use cows milk too, it is best with all cream left in. When cooking the milk cook over a low/medium heat it will taste better the slower you cook it.
